How Can I Obtain Hospice Services?

You should start by speaking with your Physician. However, anyone can request information about Hospice by contacting our office. It is the patient's right to choose which Hospice agency they wish to provide services. 

Criteria for Admission: 

The Hospice diagnosis relating to a life limiting illness or an End - Stage Disease process such as: 

Mature Couple Showing Affection
  • End Stage Cardiac Disease

  • End Stage Neurological Disease

  • Alzheimer's Disease/Dementia

  • Parkinson's Disease/ALS

  • Renal Failure/ Kidney Disease

  • Stroke/ CVA

  • Frailty

  • End Stage Pulmonary Disease

  • Cancer

  • Blood Disorder

  • HIV/AIDS

Services Provided:
  • Registered Nurse on call 24 hours/day, 7 days a week.

  • Medical Directors.

  • Pain and Symptom Management.

  • Personal Care Assistance.

  • Therapies to increase safety and comfort.

  • Social Worker and Pastoral Care.

  • Medication and Equipment Assistance.

Who Pays for Hospice?

Solace is fully funded by Medicare, Medicaid and private insurance. All of our services provided are Medicare and Medicaid regulated.
